Data Request Form
Click “ABA GO Login” in the top right corner of our website to access the portal. On the login page, you will need to enter your user ID, such as your eight-digit ABA ID, Social Security Number or email address and password.
If you forgot or have never created a password, click “Forgot password?” on the login page. You will need to enter the last four digits of your social security number along with two additional fields, either your ABA ID, date of birth (MM/DD/YYYY) or your last name. Then, click “Continue.” Your password must be seven or more characters long and contain a capital letter and a number.
To obtain a receipt, please log into your ABA GO portal account. Once you are logged in, click on the link near the top titled “Personal Info” and then the link “Receipts.”
You can request a printed initial, subspecialty or MOCA certificate via your ABA GO portal under Account Settings > Add On Services.
